Suffolk Community Healthcare is an innovative and transforming provider of community health services within Suffolk. Our vision is for a flagship service to support the shift of higher acuity care from acute hospitals into the community setting. This includes clinical interventions that will prevent hospital admissions and that will facilitate patients being cared for earlier in their recovery following hospital admission in the community setting.
We are looking for a motivated Occupational Therapist to join the Mildenhall Community Healthcare Team on a part time basis.
As a Team of District Nurses, Physiotherapists & OT`s, we cover the towns of Mildenhall and Brandon along with the surrounding more rural villages, working closely with GP`s & other agencies.
A passion and commitment to work with patients in the community & avoid hospital admission where possible is required.
Successful candidates must
be able to demonstrate that they can undertake appropriate assessments of patients in their own homes, develop and manage care plans and deliver treatment within a multi-disciplinary team.
This is an exciting opportunity to enhance your skills with the support of experienced Therapy and Nursing staff.
A car driver is essential.
-Diploma or Degree in Occupational Therapy /Physiotherapy
-Health Care Professionals Council Registration
-Planning and decision making skills
-Knowledge and understanding of professional and ethical issues
-Current knowledge and skills of moving and handling dependent adults and older people
-A knowledge of primary and community services
-Knowledge and understanding of the NHS Plan and NSF`s.
-Experience of practice evaluation
-Prior work/voluntary experience in associated field
-Wide range of fieldwork placement during training including experience of working with adults with complex physical, sensory and cognitive disabilities
-Be able to work weekends and Bank Holidays
-Ability to travel to rural locations
